Rework ttm tt swapin/swapout logic.

Rather than handling `swapping in/out' here, per se, we let uvm do
that, we interpret `swap out' as `deactivate pages', and we add
generic ttm operations to wire and unwire pages, for the ttm_tt
driver to use.

This fixes certain graphics buffer eviction logic, which enables
nouveau to suspend/resume on one of my machines. (The machine
doesn't resume overall for other reasons, but the nouveau device
suspends and resumes in isolation.)

XXX pullup to netbsd-7 after a couple weeks

provide a ttm-backed set of nouveau_uvm_ops{} in the ttm_bo_driver{}.
fixes NULL pointer derefs inside UVM due to lack of pager ops.

fix various problems i've seen where cv_*wait*() return ERESTART,
which is -3 in netbsd, which we have mapped linux ERESTARTSYS to.

this has a problem because linux code often returns errors and
pointers in the same value, and pointer values between -4095 and
-1 are considered as error returns, but -3 ends up as 3 and thus
is not considered an error, and mayhem ensues.

with this in place my kabylake system seems actually stable, i
have not triggered any of my prior issues in almost 4 weeks now.


Taylor asked me to write up a description and then wrote most of
the text below for me :-)


In Linux code, we always work with ERESTARTSYS so the code meaning
start over is a positive NetBSD errno safe for PTR_ERR/ERR_PTR.
To achieve this:
1. adapt all cv_waits that return to Linux so they map ERESTART to
ERESTARTSYS, and
2. adapt all returns to userland so they convert ERESTARTSYS to
ERESTART.
Leave EINTR and all other error codes alone.

Expose nvif ioctl interface.

nvif is a variable length (nested..) ioctl, so it doesn't match the
usual drm_ioctl interface. linux uses a shim to override the ioctl
function for nouveau to allow this, do the same.

fixes 3D acceleration with nouveau.

Draft support for drm prime.

Allow nvkm_client_map to map subregions.

Linux ioremap does not care about overlapping mapped regions, but
bus_space_map does. Since nouveau ioremaps the entire register space
of BAR 0, and separately some subregions of it, we need to convey the
bus addresses in question to it in order to bus_space_subregion.

Kinda kludgey, but we don't care about running this in userland or
anything.

While here: initialize object->map.tag. Not physically necessary on
x86 as long as the x86_io_mem_tag is nonull, so this won't fix any
symptoms on x86, but it is wrong to leave it null.

Reduce diff a little.

This code was already #ifndef __NetBSD__; the corresponding logic
lives in nouveau_module.c.

Conditional here shouldn't be necessary now.

Ifdef out some stuff we don't need.

struct drm_bus abstraction is no more. Cope.

struct drm_driver now has the bus-specific intr establish routine
(called request_irq/free_irq to match Linux's style).

Simplify and make extensible the config_search() / config_found() /
config_attach() interfaces: rather than having different variants for
which arguments you want pass along, just have a single call that
takes a variadic list of tag-value arguments.

Adjust all call sites:
- Simplify wherever possible; don't pass along arguments that aren't
actually needed.
- Don't be explicit about what interface attribute is attaching if
the device only has one. (More simplification.)
- Add a config_probe() function to be used in indirect configuiration
situations, making is visibly easier to see when indirect config is
in play, and allowing for future change in semantics. (As of now,
this is just a wrapper around config_match(), but that is an
implementation detail.)

Remove unnecessary or redundant interface attributes where they're not

There are currently 5 "cfargs" defined:
- CFARG_SUBMATCH (submatch function for direct config)

- CFARG_IATTR (interface attribte)

...and a sentinel value CFARG_EOL.

Add some extra sanity checking to ensure that interface attributes

nouveau(4): Fix return value of fence wait.

kpause(9) returns EAGAIN/EWOULDBLOCK when it times out, which is
perfectly normal and expected here with an internal one-tick timeout
which should just go through another loop iteration. This function
is expected to return the number of ticks left to wait when done, so
it reports timeout by returning zero, and is forbidden to fail with
EAGAIN/EWOULDBLOCK.

This is also forbidden to fail with EINTR -- on interruption it must
fail with the Linux ERESTARTSYS instead. So map EINTR to that too
while here.

Allow nvkm_client_map to map subregions.

Linux ioremap does not care about overlapping mapped regions, but
bus_space_map does. Since nouveau ioremaps the entire register space
of BAR 0, and separately some subregions of it, we need to convey the
bus addresses in question to it in order to bus_space_subregion.

Kinda kludgey, but we don't care about running this in userland or
anything.

While here: initialize object->map.tag. Not physically necessary on
x86 as long as the x86_io_mem_tag is nonull, so this won't fix any
symptoms on x86, but it is wrong to leave it null.

Kludge to pass bus_space_tag through nvif mappings.

- New `ioctl' NVIF_IOCTL_V0_MAP_NETBSD returns bus_space_tag_t too.
. Blocked from userland. (Would only disclose a kernel virtual
address, so not terribly serious.)
- struct nvif_driver::map function takes bus_sapce_tag, returns
bus_space_handle in addition to kva pointer.
- struct nvif_driver::unmap function takes bus_space_tag and
