Initialize DAD components properly

The original code initialized each component in non-init functions such as
arp_dad_start and nd6_dad_find, conditionally based on a global flag for each.
However, it was racy because the flag and the code around it were not
protected by a lock and could cause a kernel panic at worst.

Fix the issue by initializing the components in bootup as usual.

nd6: restore a missing reachability confirmation

On sending a packet over a STALE cache, the cache should be tried a reachability
confirmation, which is described in RFC 2461/4861 7.3.3. On the fast path in
nd6_resolve, however, the treatment for STALE caches has been skipped
accidentally. So STALE caches never be back to the REACHABLE state.

To fix the issue, branch to the fast path only when the cache entry is the
REACHABLE state and leave other caches to the slow path that includes the
treatment. To this end we need to allow to return a link-layer address if a
valid address is available on the slow path too, which is the same behavior as
FreeBSD and OpenBSD.

Some callers of rtrequest1(RTM_ADD) adjust rt_ifa of an rtentry created by

different from requested one. It's wasteful and even worse introduces a race
condition. rtrequest1 should just use a passed ifa as is if a caller hopes so.

Use pool(9) for llentry allocations
llentry is easy to be leaked and pool suits for it because pool is usable to
detect leaks.

Also sweep unnecessary wrappers for llentry, in_llentry and in6_llentry.

Fix the root cause of the hack "disable optimizations for uvm_bio.c
on 32 bit kernels".
gcc converts a division in the calculation of UBC_UMAP_ADDR macro
to multiplication (smul or combination of add/sll), and the
register of its result contains a garbage in upper 32 bits (the
upper 32 bits of smul/add/sll's result isn't zero cleared).
Then it passes to pseg_get{,_real} through pmap_extract without the
zero clear of upper 32 bits in the optimization case. So the
result of pseg_get and pmap_extact sometimes gets screwed up.
The hack has been resolved in the revision 1.340 of
sys/arch/sparc64/sparc64/locore.s.

GDB changes:
- delete gdb53
- enable gdb6 on all architectures
- add support for amd64 crash dumps
- add support for sparc64 crash dumps
- add support for /proc pid to executable filename for all archs
- enable thread support for all architectures
- add a note section to kernels to all platforms
- support detection/unwinding of signals for most architectures.
- Fix PTHREAD_UCONTEXT_TO_REG / PTHREAD_REG_TO_UCONTEXT on sh3.
- Apply fix from binutils-current so that sparc gdb can be cross built
on a 64bit host.
SA/pthread changes:
Pre-allocate memory needed for event delivery. Eliminates dropped
interrupts under load.
Deliver intra-process signals to running threads
Eliminate some deadlock scenarios
Fix intra-process signal delivery when delivering to a thread waiting
for signals. Makes afs work again!

Add support for i2c bus on AMD-8111 used on many Opteron server motherboards,
and ADT 7463c thermal and voltage monitor found on the Tyan S2881 and S2882-D
(and probably other boards as well). We do not currently support any kind
of detection of the i2c address of the thermal monitor; it appears to be
at 0x2D on the S2881 and 0x2E on the S2882-D (kernel config examples
forthcoming).

pullup the wrstuden-fixsa CVS branch to netbsd-4:
toolchain/35540 - GDB 6 support for pthreads.
port-sparc64/37534 - ktrace firefox gives

GDB changes:
- delete gdb53
- enable gdb6 on all architectures
- add support for amd64 crash dumps
- add support for sparc64 crash dumps
- add support for /proc pid to executable filename for all archs

- add a note section to kernels to all platforms
- support detection/unwinding of signals for most architectures.

SA/pthread changes:
Pre-allocate memory needed for event delivery. Eliminates dropped
interrupts under load.
Deliver intra-process signals to running threads
Eliminate some deadlock scenarios
Fix intra-process signal delivery when delivering to a thread waiting
for signals. Makes afs work again!

Apply almost all fixes and improvements from netbsd-6 except for
the rev. 1.196's iqdrops' change.

- Add the detach code.
- Add code for WOL, ASF, IPMI and Intel AMT. WOL is disabled by default
- Add Yet another workaround for ICH8.
- 82576 is dual port, so check the FUNCID and increment the MAC address for
the 2nd port.
- Fix the names of 82577L[MC] LAN controllers (for mobile).
- Fix CTRL_EXT_SWDPIN() and CTRL_EXT_SWDPIO() macros. The bit order of the
SW definable pin is not 6543 but 3654!!!
- Rewrite the code to read MAC address from eeprom.
- Add 82580 support.
- 82571 quirk. Only 82571 shares port 0 of EEMNGCTL_CFGDONE.
- The document says that the TDH register must be set after

Don't restrict the offset when allocating a map entry for in-kernel map -
use UVM_UNKNOWN_OFFSET in the call to uvm_map_prepare.
This fixes a '"panic: malloc: out of space in kmem_map" when it's not
really' testcase of mine, and one reported to me by chuq. This is likely
to fix PR/35587 as well.
