Revert vadvise(2) removal

This system call was used in legacy Lisp code, that was inherited to modern
age and still compiled against supported compat layers (e.g. in clisp,
oaklisp, Franz Lisp).

It used to instruct the kernel about paging policy (G/C aware, flush etc).

Newly compiled code (assuming that it will detect vadvise()) will use the
libc stub for vadvise(). The headers for this interface are gone.

vadvise(2) could be marked as COMPAT_80, but as long as we support ultrix,
sunos or aout68k ABI, don't bother with this.

Convert tsleep(&lbolt) to kpause(). Make ltsleep/mtsleep on lbolt
illegal. I examined all places where lbolt is referenced to make
sure there were pointer aliases of it passed to tsleep, but put a
KASSERT in m/ltsleep() just to be sure.

When using 16KB pages in a 64 bit kernel, the amount of address space our page
table can address can be larger than the amount of address space the CPU
implementation supports. This change limits the amount address space to what
the CPU implementation provides.

New callout mechanism with two major improvements over the old
timeout()/untimeout() API:
- Clients supply callout handle storage, thus eliminating problems of
resource allocation.
- Insertion and removal of callouts is constant time, important as
this facility is used quite a lot in the kernel.

The old timeout()/untimeout() API has been removed from the kernel.

Redo the SSP wrappers to be transparent on the resulting object files.
This works by having the inline wrapper calling a second function which
uses renaming to output the correct function name.
